A simple contact form

This contact form will allow your visitors to contact you via your Web site. The script does some basic input checks (e-mail address validation, etc.) and is easy to set up and configure. The script also has the option to validate domains to help avoid misuse by spammers. This version logs the IP addresses to a log file (or e-mails you the details) of anyone who attempts to access the form from anywhere other than your domain.
